House – bill introduced to exempt Puerto Rico

clip_image006 Representative Palmer (R-AL) introduced a bill (H.R. 3966) to provide that provisions of title 46, United States Code, popularly known as the Jones Act and relating to carriage of passenger and merchandise in coastwise trade shall not apply for 5 years with respect to such carriage to and from Puerto Rico. Official text of the bill is not yet available, but Representative Palmer issued a press release explaining the measure. (10/5/17) [].

Australia – rudder failure grounding

clip_image008 The Australian Transportation Safety Bureau (ATSB) issued the report of the 19 August 2016 grounding of the tanker Bow Singapore at Port Phillip, Victoria. At 1614, as the ship neared the eastern end of the South Channel, the rudder ceased responding to helm inputs and remained at five degrees port. The ship started swinging toward the edge of the channel. Steering was regained a short time later but, despite the efforts of the pilot, the ship grounded at 1617. Investigation revealed that a telemotor solenoid, controlling the rudder’s movement, had stopped responding to electrical signals. Report 2016-005 (10/6 /17) [].

The article discusses the value of near-miss reporting in the improvement of safety and identifies a methodology that the US Coast Guard could adopt, with industry support, to implement a confidential near-miss reporting regime similar (but not identical) to the program utilized in the aviation industry.
